The minimums sections requires RVR 4000. However, Part 91 operators can't use RVR. You'll need to use the FAA's comparable values chart, and convert RVR to SM. On the comparable values chart, RVR 4000's comparable value is 3/4 SM. So you'll need 3/4 SM flight visibility to land from the approach.
